--In lieu of using the old web form, FDLP libraries should submit claims for publications on GPO-issued shipping lists directly through either:
1) Log in to askGPO [ https://ask.gpo.gov/s/ ].
---From the top menu above the tiles, pull up the drop-down list under "MORE" and click on "Claims Request Form" to access the form.
2) Email to: depositoryclaims@gpo.gov
---In your message, please include:
a) Your depository library number
b) Shipping List number
c) Item number
d) SuDocs number
e) Title
